Facility Permit Identification Upload

    ONLY SUBMIT THIS FORM AFTER YOU HAVE SUBMITTED A PERMIT.

    This form is to be used only to upload identification documentation for a previously submitted facility rental permit.

    You will need to have your permit receipt handy as you will need information that is printed on your permit receipt.

    Applicants named on permit are EXCLUSIVELY responsible for all aspects of the permit and rental process including, but not limited to, permit submittal, providing ID, payments, communications, clean up, policy compliance, key pickup and return, etc.

    The identification you are providing must match the person who submitted the permit otherwise this form will be deleted.
